Information on Toyonaka City Extraordinary Special Benefit

※Eligible Households① will be notified through postal mail from the City Office.


In order to support people's lives and livelihoods amidst the prolonged effects of the Covid-19 pandemic, a cash benefit of 100,000 yen per household will be provided to households which are exempted from resident tax, etc. Please check the following information and submit the " Confirmation of Eligibility" together with requirements using the enclosed return envelope.


<Outline>


【Eligible Households】

(1) Households in which all members of the household are exempted from the per capita portion of resident tax for the fiscal year 2021 as of December 10, 2011.

(2) In addition to (1) above, households whose finances have suddenly deteriorated due to impact of the Covid-19 pandemic, and who are deemed to be in the same situation as the households in (1) above.


【Amount of Benefit】 100,000 yen per household

【Benefit Recipient】 Head of household

【Note】 ※This is different from the "Extraordinary Special Benefit for Households with Children".


<How to Process>

・In the case that the household is (1) above,

The application must arrive no later than April 28, 2022 (Thursday).

How to Process: Please return the following documents using the enclosed return envelope or apply the Toyonaka City Electronic Application System.

You can also apply through the Toyonaka City Electronic Application System.

Please note that it may take more than one month before the benefit is sent depending on the situation of the application.


Requirements

① Confirmation of Eligibility

② A copy of an ID of the head of household (driver's license, health insurance card, etc.)

③ A copy of a document which shows the receiving bank account (bankbook, cash card, etc.)


・In case the household is (2) above


Upon application by the head of the household. (No documents will be sent from the city.) We will post detailed information on the city’s website as soon as the national policy and details are decided.


※ Please inquire separately if the application is made by proxy.


Requirements


Please send the following documents together with Confirmation of Eligibility using the enclosed return envelope (green).


A copy of an ID of the head of the household (processing・recipient)

One of the followings.

(Example)

Residence Card

Passport

Special Permanent Resident Certificate

Driver's License

Health Insurance Card

Physical Disability Certificate

My Number Card with a photo

etc. (For others, please contact us.)


A copy of the bankbook (back of the front cover) of the receiving bank account etc.

If the account is already printed on this confirmation form, you don’t need to submit the copy.

The copy should contain the bank name, branch name, account number, and account holder (in katakana).
